Early Childhood Center

Toddler Program, Preschool & Pre-Kindergarten Checklist

Preschool: Student must be three (3) by August 31st
Pre-Kindergarten: Student must be four (4) by August 31st
Schedule a Visit & Family Interview

Copy of Birth Certificate
Copy of Immunization/Health Records
Copy of any applicable report cards or progress reports
Copy of any applicable development assessments/evaluations

Elementary Grades K-5

Kindergarten Checklist

Student must be five (5) by August 31st

Copy of Birth Certificate
Kindergarten Screening (scheduled after applying
Copy of Immunization/Health Records
Copy of any applicable report cards or progress reports
Copy of any applicable development assessments/evaluations

**Children entering kindergarten must be immunized in compliance with the Massachusetts Department of Public Health requirements before October of the kindergarten year.

Grades 1 to 5 Checklist

Schedule a Visit & Family Interview
Schedule to be a Saint for a Day & spend the day in the life of a Sacred Heart Student!
Application Form
Birth Certificate
Immunization/Physical Exam/Health Records
Current Report Card/Progress Report
Testing Records
Letter of Recommendation from Current Teacher
Transcript & Disciplinary Report
Applicable Developmental Evaluations – IE, IEP, 504 Plan, if applicable
Placement Test for Grades 3, 4, 5 – Grades 1 & 2 may be requested based on student records.
